But both the theory and practice of the struggle show that such reforms can only be a by-product of the wide class struggle outside the parliamentary field and elective technologies. There is no such situation in modern Russia either. For a long time, the threat of sudden death hangs over the plant, because in March of this year the management planned to reduce 963 workers, which was preceded by a suspension of the production of refrigerators. Now the factory employs 4.3 thousand people, it is planned to reduce it to 2.8 thousand people.

The reason was announced by the Ministry of Trade of the Saratov region that the production of refrigerators does not justify itself economically. But I want to ask the current government: “But aren't you guilty of this?” Indeed, it is thanks to the policy of our elite that the citizens of Russia are getting poorer every year. Judging by the upcoming events, soon our citizens will have nothing to buy, not only refrigerators, but even their contents. A year earlier (in May 2019), it was planned to lay off 470 employees. The true reason for this is the ineffective leadership policy. But this is short-sightedness or deliberate murder of the plant - decide for yourself! It is interesting that SEPO production facilities allow the production of 600 thousand refrigerators per year, but in factonly a hundred thousand units leave the conveyor. And not surprisingly, because the production equipment has not been updated almost since the 90s.

At first, enormous amounts of money “swelled” into the industry. At the expense of the federal budget, hospitals then began to receive a large amount of equipment, mainly of foreign manufacture. Large private clinics and many offices dealing with the supply of medical equipment from abroad appeared. Large foreign manufacturers also began to open branches in the country.
The opportunities for "cutting the dough" were then simply fabulous. One certification of medical devices was worth it. Roughly speaking, the difference between medical scissors and scissors, which we can buy in any hardware store, was only that some have a certificate confirming their compliance with the requirements for medical equipment, while the others do not. The difference in price was impressive: scissors purchased from an official supplier could cost dozens of times more than the same, but not certified.
Together with foreign equipment, the flow of drugs from abroad has also increased. They gradually replaced domestic counterparts: then only about a dozen factories for the production of medicines remained on the territory of the Russian Federation. And now even less.

It was a short period of industry growth due to the infusion of funds. Equipment was updated, doctors began to actively study: learn foreign languages, ride on international conferences. Those who have successfully mastered, studied new equipment, began to do operations faster, better, better. And slowly began to earn.

At the same time, the in-house production of medicines and medical equipment did not develop at all, the calculation was only for import. In people, the stereotype “domestic means bad” was actively formed.

At first they promised to pay extra for this, but then these “crumbs” were cut off with various frauds. And the load on each nurse increased by 3-4 times.

Anyone who is even a little familiar with the organization of health care in the country will understand that this is not as simple as it might seem at first glance. After all, a doctor practicing in the territory of the Russian Federation must have an education corresponding to Russian standards, confirmed by a patent authorizing work in this area. He should be able to prescribe treatment in accordance with Russian standards, which are largely different from foreign standards. And he must know the patient’s language at that level in order to be able to correctly and easily understand his complaints.

Therefore, attracting migrant workers is unlikely to go smoothly and will be able to at least to some extent solve the issue of a shortage of doctors in the Russian Federation. This idea is more like another attempt to put pressure on doctors and force them to accept the imposed conditions of the game.

The crisis of medical education
Negative changes affected not only the work of doctors, but also the medical education system. They are closely related to what happened in the education system of the Russian Federation after the collapse of the Union, but there are a couple of points that I would like to highlight.

Particularly clearly visible failures in the medical education system on the example of surgery. Surgical specialties require the development of fine motor skills of the hands. It is impossible to learn from one book, you need a lot of practice.

Until about 2005, the remnants of the Soviet school still existed: at universities there were still anatomical theaters in which students saw what real human organs looked like, with which they would later have to work. After all, otherwise, without touching them, without feeling them with your fingertips, it is impossible to learn how to operate.

In 2005, it was decided that such an approach was expensive and not modern. It is much easier to learn the theory from textbooks, and to create 3D models for visualization. Indeed, in addition to the purely financial issue, which in itself is quite large in modern conditions, a number of legal difficulties are associated with the work of anatomists. The anatomical theater is, first of all, the bodies of dead people. They need to be taken somewhere, while formalizing everything officially at the state level.

In the USSR, all this passed through the Medical Institute and through the Medical Anatomical Theater. And all universities had a base in the form of hospitals in which they had their own morgues. There, students could observe the autopsy and even participate in it themselves. And the bodies of the dead, which were not taken away by relatives, were given to the anatomical theaters for a certain period before the funeral at the expense of the state.
In the modern world, different states solve this problem differently. Illustrative, for example, is the approach of the Gdansk Medical Institute, in which there is an anatomical theater. His leadership offers citizens for a certain amount of money to bequeath their body after death to the anatomical theater for three years. After the expiration of this period, a person is buried at the expense of the institute and for a long time they look after the grave. Similar is found in Europe.
There is no such practice in Russia. Therefore, in medical education, failure gradually forms. First at the level of universities, and then already in residency.

The resident is a specialist who graduated from the institute and received a doctor’s diploma, but has not yet mastered the surgical specialty in practice. Based on modern Russian realities, this may be a person who did not hold a real surgical instrument in his hands and could not do anything. For such a specialist for 2 years of residency at one of the hospitals, it is necessary to master the chosen specialty in practice.

But the modern realities of Russian hospitals are often layered on the training process here. The doctors, loaded upside down with paperwork, instead of actively including beginners in practice, try to shift the paperwork to them as much as possible. And as a result of this approach, after 2 years of residency, a certified surgeon with the level of knowledge and skills much lower than necessary leaves the hospital walls. The best that such a specialist can do is work in the hospital where he studied and where he managed to learn the "kitchen".

Is there a future?

The disadvantages of education are superimposed on top of the existing problems of the industry, and the result is an unattractive picture.

To the low salaries of doctors and nurses and poor material support is added a huge amount of paper work. If we take, for example, the modern history of the disease, which is started for each patient, then the first 15 - 20 pages are not related to treatment at all. These are various statistical coupons, consent to the processing of personal data and various manipulations and the like red tape.
In addition, quite a strict legal framework has now been established related to the work of doctors and medical staff. Pay attention to criminal cases that are opened in connection with various medical errors. Many are surprised that doctors are silent, not saying anything in their defense. But the thing is that they have no right to speak, because they are connected by the law on the protection of medical confidentiality. That is why it is very difficult for doctors to answer numerous accusations against them.

It is logical that few experts want to work in such a system of their own free will. Therefore, most doctors are now drawn to private clinics. And he lures for those patients who are able to pay for treatment so as not to mess with the current system of compulsory medical insurance (MHI) in the country, according to which a person can wait for the required procedure for weeks, or even months.

And this compelled craving for paid medical care is very beneficial to the capitalist system that has developed in the Russian Federation. By deliberately aggravating it in this way, the state relieves itself of the remnants of the obligations for social protection and support of its citizens, which are still preserved from the times of the USSR.

If events continue to develop in this way, the time is not far off when healthcare in the Russian Federation will become completely paid, and therefore inaccessible to a significant part of the population. As in the USA or Great Britain, which for liberal minds are persistent role models.

The situation with coronavirus in these countries shows us how such an organization of medicine is able to cope with any serious epidemiological threats.
